Finally got it.. as part of Sony Bravia PWP (Purchase with Purchase) during Sony Days 2016. For my Sony Bravia model, it cost $199 to PWP the Sony Playstation 4 (500GB model).
The process is simple.
(1) Buy the TV
(2) On the day of delivery, go to the Sony web site and register your TV. Make sure the purchase date of the TV (that you entered into the warranty web site) is the same as the invoice date. This is to ensure that you fall into the period of the PWP
(3) Wait. I waited about 5 working days. You will then receive an email about the redemption of the PWP product. Print the email.
(4) Go down to the Sony centre (313 Somerset). Produce the email. Produce the invoice.
(5) Pay $199
(6) Viola

The accessories included the power supply, HDMI cable, USB charging cable for the Dual Shock 4 Controller, an ear piece (not sure how to use that yet). Network cable not provided 🙂
